Infusion & Hematology Referrals
At Whitman Hematology & Infusion Therapy, we are honored to partner with referring providers to deliver high quality, patient centered care. To ensure a smooth, efficient, and timely referral process, we ask that all required documentation be submitted in full. Incomplete referrals may result in delays in scheduling or treatment.
Infusion Therapy Referrals
To initiate an infusion therapy referral, please provide the following:
1. Provider Order Form: Obtain Here
  • Signed order from the referring provider
  • Medication name, dosage, frequency, and duration
  • Route of administration
Patient Demographics
  • Full name, date of birth, address, and phone number
  • Emergency contact information
Insurance Information
  • Clear copies of the front and back of all active insurance cards (primary and secondary, if applicable)
Diagnosis Information
  • Relevant ICD-10 code(s) supporting the infusion therapy
Clinical/Progress Notes
  • Recent provider notes supporting the primary diagnosis and medical necessity of the prescribed treatment
Supporting Labs or Diagnostic Tests
  • Relevant lab results, imaging, or other diagnostic reports that substantiate the diagnosis and treatment plan

What to Expect

Once we receive a complete referral packet:
  • Our team will verify insurance benefits and obtain prior authorization if needed.
  • We will contact the patient directly to schedule their first appointment.

Hematology Referrals

At Whitman Hematology & Infusion Therapy, we work closely with referring providers to ensure patients receive timely, comprehensive hematology care. To facilitate an efficient referral and enable our physicians to provide the most informed evaluation, we ask that the following documentation be submitted prior to scheduling an appointment:

Required Documentation

  • Referral from the patient’s current physician
  • Clinical notes relevant to the patient’s diagnosis
  • Imaging reports (CT, MRI, ultrasound, etc.)
  • Laboratory results and other diagnostic tests, including genetic testing or bone marrow biopsy reports and slides, if applicable
  • Patient demographics (full name, date of birth, contact information)
  • Insurance information (front and back copies of all active insurance cards)

Next Steps

Once the records are received and reviewed, our Referral Specialists will contact the patient directly for appointments and to coordinate any necessary follow up.
